Registration of children under 14 (hereinafter "children") is done through separate forms written in plain language that children can understand, and consent from legal representatives is mandatory when collecting personal information.
The Clinic collects minimal information such as the legal representative's name and contact information from children to obtain consent, and obtains consent from legal representatives according to methods specified in the Privacy Policy.
Legal representatives of children may request to view, correct, and delete children's personal information. To view, correct, or delete children's personal information, click Member Information Modification, go through the legal representative verification process, and directly view, correct, or delete the child's personal information, or contact the Privacy Officer in writing, by phone, or fax for necessary action.
The Clinic does not provide or share information about children with third parties, and prohibits the use and provision of personal information collected from children until errors are corrected when legal representatives request correction of errors.
※ Personal information required to be retained by law cannot be modified or deleted even upon request during the retention period.
